What it is

Metformin is a biguanide antidiabetic drug derived from galegine. FDA-approved since 1995 for type 2 diabetes. Extensive clinical experience including in non-diabetic populations.

Evidence snapshot

Category: Longevity. Status: Human Trials. See the references section below for the underlying primary sources.

Mechanism

Primary mechanism is suppression of hepatic gluconeogenesis via AMPK activation and mitochondrial complex I inhibition. Additional effects on inflammation and cellular stress responses.

Key research findings

Observational studies in diabetic populations have suggested reductions in age-related disease incidence. The TAME trial is the first major RCT designed to test metformin in non-diabetic age-related outcomes.

Limits of the evidence

Observational data subject to confounding by indication. TAME trial pending completion. GI side effects are common.
